Fournier's gangrene and associated sacral decubitus ulcer: reconstruction with superomedial fasciocutaneous flap of the thigh and myocutaneous flap from the gluteal muscle - a case report

Abstract

The aim of this work is to describe the case of a patient who developed Fournier Gangrene (FG), a necrotizing bacterial infection that affects the perineal, perianal, and genital regions. The male patient, 37 years old and suffering from diabetes mellitus, was initially diagnosed with a perianal abscess that progressed to septic shock and involvement of the perineal and scrotal regions, and later developed a sacral pressure ulcer. After clinical stabilization, he underwent a series of debridements and reconstructions of the scrotal, perineal, and sacral regions. Scrotal and perineal reconstruction was performed using a supramedial thigh fasciocutaneous flap, bilaterally, and sacral region reconstruction was accomplished with a myocutaneous flap from the gluteal muscle. The case study concluded that both techniques used proved to be excellent treatment alternatives as they provided good coverage, satisfactory aesthetic and functional outcomes, and did not result in significant sequelae in the donor area.
